We know everyone has a lot of requirements placed upon our personal and family time and sometimes we miss an opportunity to help others. To encourage member participation in our events, we are providing this new form.  Here you can tell us your interests for participating in the Council events. 

This table form conttain a composite list of the annual events conducted by the Council in the format: EVENT NAME/BRIEF DESCRIPTION/DATE & TIME(S)/LOCATION

Participation will be entered selecting 1of 3 Check Boxes - YES/NO/MAYBE
Signature will be NAME/EMAIL/MEMBERSHIP # reCAPA secured from spam/bots
System generated acknowledgement/thank-you email.

NOTE PROGRAM COORDINATOR: This is same form perspective members receive with their application in an online version. Can be used for new members and updating current membership interests.
